Clinical Team
Our clinical team provides a wide variety
of services in the home setting. Care is
customized to meet the patient's needs based
on physician orders. Services are coordinated
by a team of licensed or certified staff
which may include:
Nursing Services:
- Patient education
- Skilled observation and
assessment
- Direct care such as wound
care, catheter care, etc.
- Diabetic care
- Medication teaching and/or
administration
- Ostomy care
Rehabilitation Services:
Roper St. Francis Home Health offers one
of the largest and most qualified rehabilitation
teams in the Lowcountry.
- Physical therapy to increase
mobility, strength and training with assistive
devices
- Occupational therapy to
increase independence with activities
of daily living
- Speech and language therapy
to regain and improve communication skills
Other Services:
Medical Social Services to connect patients
and their families with community resources
and help them secure funding for health
needs and other services. Home Health Aides
are available to assist with personal care
needs such as bathing and dressing, if the
patient qualifies.

planning process. In conjunction with your
physician, we will utilize the appropriate
standards of care to teach you or your caregiver
how to care for your health care needs. |
