It can be difficult to talk about end of life issues, but the best time to plan and share your wishes with your loved ones is before you become seriously ill or impaired. Advance directives are legal documents that convey your personal decisions about end of life care to your family, friends, doctor and healthcare professionals. When planning for your care it is important to consider the types of medical treatments you would desire.
To assist you in planning, we have included several forms including the Declaration of a Desire for Natural Death, Healthcare Power of Attorney and Organ Donation. Should you become hospitalized at Roper St. Francis Healthcare we have Patient Representatives available who can provide and assist you with advance directive forms.
